ah, well, two of your guesses are actually not that far off! I was seventeen when I first ascended, so I still look like I did then, but actually I am... a little over eight hundred? I've sort of lost exact count to be honest.

it can, but it's not really frequent, and there are other factors as well, like the kind of person that you are... it's a little complicated, and in fact, the way this happened for me is sort of an extraordinary tale even when I'm from.
